PUBLIC | ICAM | Incident Causation Analysis Method

Description

The ICAM model is related closely to the work of James Reason, the renowned human factors expert and his works: Managing the Risks of Organisational Accidents (1997); and Human Error (1990) in which he states: ‘Rather than being the main instigators of an accident, operators tend to be the inheritors of system defects by poor design, incorrect installation, faulty maintenance and bad management decisions. Their part is usually that of adding the final garnish to a lethal brew whose ingredients have already been long n the cooking’.

The course is aimed at those conducting investigations at a higher level, or those who will be leading investigation teams.
